User Story :
As a patient, I want to complete a comprehensive health intake form so that my healthcare provider can understand my health history, current condition, and treatment goals.

Acceptance Criteria :

• The Health Intake Form should be accessible from the left navigation menu under “Health Intake.”
• The form should be divided into multiple sequential sections — exact section count and labels are TBD (e.g., Personal Information, Medical History, Lifestyle, Goals, etc.).
• Each section should include required fields — specific fields and validations are TBD.
• A progress indicator should display the patient’s current step (e.g., Step 3 of 18 or dynamic if sections change).
• Patients should be able to move forward and backward between steps while retaining previously entered data.
• The system should auto-save progress after each step.
• Form validation should prevent navigation to the next step until all required fields are completed for that section.
• A Summary page should display all entered information for final review before submission.

On submission :

• System stores completed data in the patient’s record.
• Show success message: “Health Intake Form submitted successfully.”
• Redirect to Dashboard after confirmation.

Notes for QA :

• Validate navigation between steps and data retention.
• Confirm auto-save works after each section.
• Verify required field validation once the field list is finalized.
• Check progress indicator updates correctly when moving between steps.
• Submit a test form and confirm data is stored and confirmation message appears.
