Update the Health Intake Form by dividing it into two distinct parts: Quick Start and Clinical Deep Dive, allowing patients to complete essential information upfront and defer detailed clinical information to a later stage. **Requirements:**  **1. Quick Start Intake** * Designed for fast completion during initial onboarding. * Collect only required and essential information. * Estimated completion time: 3–5 minutes. * Includes basic sections such as: * Basic personal information * Health snapshot * Goals * Experience * Readiness * Completion of Quick Start should allow the patient to proceed with: * Account setup * Appointment scheduling **2. Clinical Deep Dive Intake** Contains detailed clinical and medical information. **Can be completed:** * Before the appointment, or * During the appointment with the provider, or * After the appointment, as guided by the provider * Intended to support deeper clinical assessment and treatment planning. * Accessible to Providers and Patients as part of the appointment workflow. **3. General Behavior** * System should clearly indicate which intake parts are completed or pending. * Providers should be able to see intake completion status during appointments. * Users should be guided clearly on when and why to complete each part. **Acceptance Criteria:** * Intake form is clearly divided into Quick Start and Clinical Deep Dive. * Users can complete Quick Start independently during onboarding. * Clinical Deep Dive can be completed at a later stage. * Intake completion status is visible to both Patient and Provider. * User flow remains smooth without forcing full intake upfront.
