Implement reminder notifications for patients who have not signed the consent form.
Requirements:
- Send reminder emails for pending consent.
- Show in-app notifications until consent is signed.
- Stop reminders automatically once consent is completed.
- Ensure reminders are not duplicated unnecessarily.
Acceptance Criteria:
- Patients receive reminders for unsigned consent.
- Notifications stop after signing.
- Providers are not required to manually follow up.

Once the user registers themself as a patient or is added by admin, the system creates a consent form based on the previously decided template and sends email and in-app notifications informing that the consent form is ready to review and sign.

QA test cases :
- Verify consent form is automatically created when a patient self-registers or is added by Admin using the configured template.
- Verify one-time email notification is sent informing the patient that the consent form is ready to review and sign.
- Verify one-time in-app notification is sent informing the patient that the consent form is ready to review and sign.
- Verify no reminder emails are sent for pending consent forms.
- Verify in-app notification remains visible while consent status is Pending.
- Verify in-app notification stops automatically once the consent form is signed.
- Verify no duplicate email or in-app notifications are generated unnecessarily.
- Verify consent status updates to Completed after successful signing.
- Verify providers are not required to manually follow up for unsigned consent forms.
